Preuss Resident Research Award: maximal resection of low-grade intrinsic brain tumors using "awake" craniotomy and multiple marginal smear biopsies: neurological deficit rate and long-term survival data.

چکیده
INTRODUCTION “Awake” craniotomy with multiple marginal biopsies was used to resect 154 presumed low-grade brain tumors. Intraoperative neurological deficits were more likely to be permanent (47.1%) than those detected postoperatively (12.5%). The permanent, significant neurological deficit rate was 6.5%. The survival rate for Grade 2 lesions was 65.1% (43 patients) at 10 years and 85.9% (71 patients) at 5 years. Survival was better after total resection.

Initial Experience with Brain Mapping under Awake Craniotomy for Resection of Insular Gliomas of the Dominant Hemisphere
Background & Importance: Insular lobe is located at the depth of sylvian fissure and is hidden by frontal, temporal and parietal lobes in close vicinity of internal capsule and basal ganglia and adjacent to the speech centers in the dominant hemisphere. Thus, radical resection of insular gliomas can be even more difficult. Brain mapping techniques can be used to maximize the extent of...
متن کاملAwake craniotomy vs surgery under general anesthesia for resection of supratentorial lesions.
BACKGROUND The use of an awake craniotomy in the treatment of supratentorial lesions is a challenge for both patients and staff in the operation theater. OBJECT To assess the safety and effectiveness of an awake craniotomy with brain mapping in comparison with a craniotomy performed under general anesthesia. METHODS We prospectively compared 2 groups of patients who underwent surgery for su...
متن کاملLong term reshaping of language, sensory, and motor maps after glioma resection: a new parameter to integrate in the surgical strategy.
OBJECTIVES To describe cortical reorganisation and the effects of glioma infiltration on local brain function in three patients who underwent two operations 12-24 months apart. METHODS Three patients who had no neurological deficit underwent two operations for low grade glioma, located in functionally important brain regions. During each operation, local brain function was characterised by el...
متن کاملAwake craniotomy under local anaesthesia and monitored conscious sedation for resection of brain tumours in eloquent cortex--outcomes in 20 patients.
INTRODUCTION Resection or even biopsy of an intra-axial mass lesion in close relationship to eloquent cortex carries a major risk of neurological deficit. We review the safety and effectiveness of craniotomy under local anaesthesia and monitored conscious sedation for resection of mass lesions involving eloquent cortex. MATERIALS AND METHODS We performed a 3-year retrospective review of patie...
متن کاملFunctional Cortical Mapping Using Subdural Grid Electrodes in Patients with Low-Grade Gliomas Presenting with Seizure
Functional Cortical Mapping Using Subdural Grid Electrodes in Patients with Low-Grade Gliomas Presenting with Seizure P low-grade brain tumors in eloquent cortex are challenging lesions to resect. The margin between infiltrating tumor and normal white matter is often indistinct, and their removal risks significant postoperative disability. Several mapping techniques have been developed to limit...
